Metatarsalgia

What Is Metatarsalgia?

Metatarsalgia is pain and inflammation in the ball of the foot, the area between the arches and toes on the bottom of the foot. The pain of metatarsalgia typically centres under one or more of the five bones at the bases of the toes, known as the metatarsal heads.

Although not often serious, metatarsalgia can be very painful. At home treatments such as ice and rest often relieve symptoms, and wearing proper footwear with shock absorbing insoles or arch supports may prevent or lessen future problems.

Metatarsalgia is sometimes confused with Morton's neuroma, which also causes pain in the ball of the foot. The key difference is that Morton's neuroma involves nerve irritation, typically between the third and fourth toes, while metatarsalgia refers to broader pain and inflammation across the metatarsal heads. Both conditions can benefit from orthotic support.

Common Symptoms of Metatarsalgia

Symptoms may include:

  • Sharp, aching, or burning pain in the ball of the foot
  • Pain that worsens with standing, walking, running, or flexing the feet
  • Discomfort that improves with rest
  • Sharp or shooting pain, numbness, or tingling in the toes, or a feeling of having a pebble in the shoe.
  • Pain that is worse on hard surfaces or when barefoot
  • Tenderness across the ball of the foot

Most often, the pain develops over several months rather than suddenly. If symptoms persist or worsen despite rest and footwear changes, it is worth seeking professional medical advice.

What Causes Metatarsalgia?

Sometimes a single factor leads to metatarsalgia, but more often several factors are involved.

High-Impact Activity Distance runners are at particular risk because the front of the foot absorbs a great deal of force during running. Anyone who participates in high-impact sport is at risk, especially if shoes fit poorly or are worn down.

Foot Shape and Deformities Certain foot shapes increase the risk of metatarsalgia. High arches concentrate load on the metatarsal heads, while flat feet can alter weight distribution across the forefoot. Conditions such as bunions and hammertoes can also shift excess pressure onto the smaller metatarsals.

Unsupportive or Ill-Fitting Footwear Wearing shoes that are too tight or too loose can contribute to metatarsalgia by concentrating pressure on specific areas of the forefoot rather than distributing it evenly.

Excess Body Weight Additional body weight increases the load placed on the forefoot with every step, raising the risk of pain and inflammation in the metatarsal heads over time.

Inflammatory Conditions Inflammatory arthritis, such as rheumatoid arthritis or gout, can also contribute to metatarsalgia.

How Orthotics May Help Metatarsalgia

Orthotics can play a significant role in shock and pressure reduction to the forefoot structures when walking, running, or jumping, making them a widely accepted treatment for metatarsalgia.

Benefits may include:

Forefoot Pressure Relief Metatarsal pads work by gently lifting and supporting the transverse arch of the foot, reducing excessive pressure under the metatarsal heads. The result is less pain, less burning, and improved comfort with each step.

Even Pressure Distribution A metatarsal pad helps ensure that pressure is efficiently distributed so that no single point of the foot is carrying more weight than it can handle, while also helping to promote the natural positioning of the padding under the ball of the foot.

Shock Absorption Foot orthoses are frequently used in sports for the treatment of overuse conditions including metatarsalgia, with one important aim being to reduce plantar pressure under prominent areas like metatarsal heads.

Arch Support Wearing proper footwear with shock-absorbing insoles or arch supports may prevent or minimise future problems with metatarsalgia by improving overall foot mechanics and reducing the concentration of load at the forefoot.

While orthotics are not a cure for metatarsalgia, they can form an important part of a broader management plan alongside rest, footwear changes, and professional guidance where needed.

Who May Benefit From Orthotics?

Orthotics may be suitable for people who:

  • Experience burning or sharp pain in the ball of the foot
  • Participate in running or high-impact sports
  • Have high arches, flat feet, bunions, or hammertoes contributing to forefoot pressure
  • Spend long hours walking or standing on hard surfaces
  • Need additional forefoot cushioning and pressure relief in everyday or athletic footwear
  • Want to reduce the risk of metatarsalgia recurring after recovery

SOLE5 Orthotics for Metatarsalgia

SOLE5 orthotics are designed to provide structured support and cushioning while fitting comfortably inside a wide range of footwear.

Key features include:

  • Forefoot cushioning to reduce pressure on the metatarsal heads
  • Arch support to improve overall foot alignment and weight distribution
  • Shock absorption to reduce impact forces through the ball of the foot
  • Heel stabilisation for consistent foot positioning during activity
  • Lightweight construction suitable for daily and athletic wear

Whether you're recovering from ball of foot pain or looking to reduce the forefoot pressure associated with high impact activity, orthotics may help make each step more comfortable.
